I would like to take a moment to introduce myself and our services to you. My name is Dr. Gabriel Nuriel and I am the owner of Primary Care of Central Florida, Inc. We are a team of licensed Doctors, Nurse Practitioners, and Physician Assistants who provide primary care services to the homebound geriatric population in the Skilled Nursing, Assisted Living, Home and Hospital Settings. We have been happily caring for our Central Florida elderly for more than 10 years. Our experienced team of professionals understands that at some point in time you may experience challenges in your health condition and find yourself in the hospital, skilled nursing or moving forward into assisted living. During this time, our group of caring and competent practitioners are ready to care for you! We provide you and your family with peace of mind knowing we offer our primary care services in each setting.

Excellent care is our top priority. As a primary care office for the homebound geriatric patient, we have a low “return to hospital” rate. Our company works with a number of ancillary services such as in-home labs, imaging, durable medical equipment, sleep studies and home health nurses, speech and physical therapists and social workers to help keep you healthy and out of the hospital.

Your privacy is a also top priority. You will be pleased to learn that medical records for our homebound patients are available on-line via HIPAA compliant secure access.

If you or your loved one is unable visit his or her primary care doctor due to a medical condition or illness, unsteady gait, or no transportation, you may qualify to have one of our providers come to your home for PCP services.
