Excellent Care

Excellent care is our top priority. As a primary care provider for homebound geriatric patients, we maintain a low hospital readmission rate. Our company collaborates with ancillary services such as in-home labs, imaging, durable medical equipment, sleep studies, home health nurses, speech and physical therapists, and social workers to help keep you healthy and out of the hospital. Your privacy is equally important to us, with medical records for our homebound patients available online via HIPAA-compliant secure access. If you or your loved one is unable to visit a primary care doctor due to medical conditions, unsteady gait, or lack of transportation, you may qualify for home visit services from one of our providers. We are here for you

We truly understand that direct oversight and management of your patients in the post-acute setting is imperative to quality patient care. Our Risk Management department includes knowledgeable Doctors, ARNPs, LPN, and Social Workers. We are able to give your patient and your MSO a smooth transition back to your PCP services.

We will happily assist you and your team with the necessary communication and follow through to provide a streamlined direct admission process for both the acute and post-acute setting.

Direct Admission Goals: Smooth, Safe Transitions

1. Avoid hospitalization if possible. 2. Communication with Hospitals, SNF buildings, PCPs and health plans. 3. 3008 completion. 4. Assistance with PASRR documentation. 5. Intermittent status reports to PCP and Health Plans. 6. Safely Transition patient back to your PCP services.
